This is underrated PC horror / FPS is notoriously unoptimised. Even the best machines are not able to run it smoothly.
These are the optimal settings for mid to high level setups:
Under Video:
Vsync: On
Shader Model: 4.0
Hardware Physics: On
Advanced PhysX: On (causes crashes)
Motion Blur: On
Camera Motion Blur: Off
Water reflections: On
Water Caustic: On
Antialiasing: Off
Under Audio:
Audio Driver: OpenAL
EAX: Off
